Facial Fillers for Facial Rejuvenation

What to use and what not. Do not be fooled.
Doctor Luis Carlos Moreno

One of the changes produced by facial aging is the loss of volume in the area around the eyes, on the cheeks, in the temple area, with the consequent formation of grooves on the cheeks,  known as nasolabial folds, and grooves between the corner of the mouth and the chin, known as marionette lines. If the facial flaccidity that accompanies the loss of volume is not important or is very slight, the use of fillers or fillers are a non-surgical alternative to hide that lack of volume and the furrows that they produce.

Around the 1980s, fillers were developed that were permanent because they contained various concentrations of medical grade silicone. Towards the year 1992 the FDA in the United States noticed that the silicone applied freely in the fillers produced a reaction in the tissues, producing marked fibrosis and deformities, for which it issued a warning prohibiting this type of fillers. Based on that warning, in all countries the use of facial fillers containing non-absorbable particles has been prohibited and only the use of products that are completely absorbable and that will therefore be eliminated from the body over time is authorized.

Incredibly in Panama there are still aesthetics, beauty salons and even worse, foreign personnel who come to the country and inject, under the deception that they are putting hyaluronic acid, silicone substances and especially silicone that is not of medical grade, with which the reactions they produce are even worse, causing true deformations of the patients' faces.

These non-biocompatible materials when injected initially look good, but since they are not biocompatible, they trigger in your body a reaction of your immune system that seeks to counteract the inflammatory effect that these substances produce in the tissues. Over time, this inflammatory reaction creates a fibrous reaction that surrounds the product and that grows over time, deforming the parts of the body where they were applied and creating hardening and deformity that is not reversible.  This process of deformation and reaction of the body to the foreign substance has become a true disease that today is known as Iatrogenic Allogenesis.

But the most serious thing is that the reaction of the immune system produced by these foreign substances can lead the patient to develop systemic diseases such as lupus erythematosus, multiple sclerosis, arthritis deformans or a number of immunological diseases that seriously compromise the health of patients. Therefore, do not be fooled and make sure that you are in the hands of a suitable professional and that the product injected does not contain silicone or any other substance foreign to the body. Take care of your health and your life.

Of the fillers or fillers that are allowed to be used, they are only Hyaluronic Acid, which is the generic name. There are different trademarks such as  perfectha, juvederm, teosyal,  restylane, balotero, among others, and calcium hydroxyapatite (Radiesse, Renú). These are biocompatible products that are authorized to be used as fillers and whose effects will go away as the body dissolves or disappears.

The fillings have different presentations.  There are hyaluronic acids for deep, intermediate, superficial and intradermal fillers.  It all depends on the preparation (crosslinked) what will make the product denser or less dense and it is also used for different areas of the face.  The results with these products are excellent, and the duration ranges from 8 months to 18 months depending on the type of hyaluronic acid and the area where it is applied. These fillers can be applied in the office but always keeping the appropriate aseptic and cleanliness measures.  The effect of hyaluronic acid, in case you are not happy with the result, can be reversed using hyaluronidase, an enzyme that digests and breaks down hyaluronic acid.

Another type of facial filler is what is known as Lipo facial fat injection, which consists of extracting fat from your own body and after preparation and decantation, injecting it into the areas where you want to increase volume. This procedure has the advantage that it uses its own fat, it is completely natural, and its result is much greater, lasting many years.  The disadvantage is that it must be performed in a surgery room, although it can be under local anesthesia, but for safety, it must be done in a sterile environment.
